After August 15, 2010, new registrations will no longer be accepted.

The program is designed to contain 3 stages that are designated for a maximum of 20 AMA PRA Category 1 Credits™.

Stage A: (Baseline Assessment) - Please complete either:
Screening Log - Baseline Form or the Baseline Assessment Survey.
   
Stage B: (Implementation) - Please review either:
Implementation Period Resource Toolkit (IPRT) and complete the Screening Log: Implementation Form, or Review "Tools/Resources" and integrate these in your practice. Both require an implementation period of 12 weeks after which you will be asked to complete the Utilization of Resources Survey.
   
Stage C: (Close-out) - Based on the option chosen in Stage A, you will be asked to complete either a Post-Activity Survey or the Follow-up Survey
